Hoist User Acceptance

Purpose 

Rebound WA provides access to the Molift 150 portable hoist to assist eligible members with transfers between their wheelchair and sporting equipment within Stirling Leisure - Mirrabooka. 

Use of the hoist is restricted to Rebound WA members who have had this application to utilise it approved by Rebound WA. 

Access to the hoist is provided subject to the conditions outlined below. 

This document is a summary of the conditions of use and is not intended to replace an operators own manual handling certification or safe use protocols.  

By completing the application for use of the hoist a user, and anyone who operates the hoist on their behalf, takes full responsibility for their own safety. 

Rebound WA are not liable for an accident, injury, damage or death that arises from use of the hoist. 

The person completing this agreement and using the hoist is solely responsible for the safety of themselves, hoist operators, other people, the hoist and all other surrounding equipment and structures. 

1. Registration and Annual Fee 

To register as an authorised user of the Rebound WA Molift 150 hoist: 

• The member must complete this required registration and acknowledgement process. 

• An annual January to December user fee of $120 applies. 

• Where registration occurs after January, the fee may be pro-rated for the remainder of the relevant year, at Rebound WA's discretion. 

• Once registered, the member may use the hoist as required while on-site, subject to these conditions and any additional requirements imposed by Rebound WA. 

Rebound WA may suspend or withdraw access to the hoist where there are concerns regarding safe use, equipment condition, training, or compliance with these conditions. 

2. Trained Operator Requirement 

The member submitting this application is responsible for ensuring that the hoist is only operated by a person who is appropriately trained and competent in manual handling and the safe operation of the hoist. 

The member must not ask an untrained or unsuitable person to operate the hoist on their behalf. 

The member is responsible for ensuring that the person operating the hoist for them understands and follows: 

• the Molift 150 manufacturer's instructions; 

• the relevant sling instructions; 

• Rebound WA's hoist safety procedures; 

• appropriate manual-handling practices; and 

• these conditions of use. 

3. Responsibility for Safe Use 

The member and the person operating the hoist on the member's behalf are responsible for ensuring that the transfer is undertaken safely. 

This includes responsibility for: 

• assessing whether the proposed transfer is appropriate; 

• ensuring the correct sling and equipment are used; 

• ensuring the hoist is suitable for the transfer; 

• ensuring the hoist and sling are checked before use; 

• following appropriate manual-handling procedures; 

• ensuring the wheelchair, sporting equipment and surrounding area are appropriately positioned and secure; 

• following the manufacturer's instructions and Rebound WA procedures; and 

• stopping the transfer if a safety concern arises. 

The member and operator must not undertake a transfer where they are unsure that it can be completed safely. 

4. Equipment Safety and Reporting 

The hoist must be checked before use and must not be used if there is any fault, damage, unusual operation or other issue that may affect safe use. 

Any issue with the hoist, sling or associated equipment that may affect safety must be reported to Rebound WA as soon as it is identified. 

This includes, but is not limited to: 

• visible damage; 

• frayed or damaged slings; 

• unusual noises; 

• problems with the controls; 

• battery or charging issues; 

• problems with the wheels or legs; 

• damaged attachment points; or 

• any unexpected behaviour during operation. 

If there is any doubt about the safety of the equipment, do not use it until Rebound WA has been notified and the equipment has been assessed. 

5. Safe Working Load 

The Molift 150 has a Safe Working Load of 150 kg. 

The hoist must never be operated above its Safe Working Load. 

Where the safe working load of the hoist, sling bar or sling differs, the lowest applicable safe working load must be observed. 

6. Use of the Hoist 

The hoist is to be used only for its intended purpose of safely transferring a person. 

Operators must: 

• follow the manufacturer's instructions; 

• use an appropriate and compatible sling (users may provide their own if possible); 

• use the hoist only for transfers they are trained and competent to undertake; 

• plan the transfer before commencing; 

• maintain a safe environment around the hoist; 

• never leave a person suspended and unattended; and 

• stop immediately if the transfer becomes unsafe. 

The hoist must not be modified or used in a manner inconsistent with the manufacturer's instructions or Rebound WA procedures. 

7. Storage After Use 

After every use: 

1. The Molift 150 must be returned to its storage location. 

2. The hoist must be stored appropriately and clear of the access point. 

3. Storage must be closed and secured after the hoist has been returned. 

The hoist must not be left unattended elsewhere on-site. 

8. Reporting Incidents or Problems 

Any accident, near miss, injury, equipment failure or other safety concern involving the hoist must be reported to Rebound WA as soon as practical via admin@reboundwa.com  

Users must cooperate with Rebound WA if further information is required following an incident or equipment concern. 

9. Acknowledgement

Anyone wishing to use the hoist must complete the Hoist User Acceptance Form below and have their request approved by Rebound WA.

Users under 18 must have a suitable parent or legal guardian complete the form. It is the responsibility of that parent or legal guardian to ensure anyone operating the hoist for the member under 18 be suitably trained to do so safely.
